- We come to you. Private homes, your home, assisted living, group homes, skilled nursing facilities. Same team, same care, anywhere in Maricopa County.
- You decide where home is. We have never asked a family to relocate a loved one to receive Sapphire care, and we never will.
- If your family ever chooses a move, we handle it with you. Some families decide a care community is the right next step. Our social workers help you find the right place, manage the paperwork, and move the care plan with your loved one so nothing is interrupted.
- We partner with the place your loved one lives. Assisted living, group home or skilled nursing: we work alongside their staff as one team, adding our nurses, aides and support on top of the care they already provide.
