Care comes to you.
Sapphire Hospice serves patients wherever they call home across the Phoenix metropolitan area. There is no building to move into. Our team comes to the patient, and the decision about where home is always belongs to the family.
In facilities, we work alongside the staff as partners: one team around the resident. And when a family chooses a care community for the first time, our social workers help find the right place, assist with ALTCS and other paperwork, and move the plan of care with the patient so nothing is interrupted.

Social Work & Benefits Navigation
Advance directives, family meetings, and help with the paperwork families dread: ALTCS (Arizona Long Term Care System) and AHCCCS applications, VA benefits, and finding or changing a care home when that is what the family wants.
