• Information you give us. When you submit our contact form, we collect your name, phone number, email address, the role you select, your preferred contact time, and your message. Please do not include medical details or other protected health information in the form.
• Information collected automatically. Like most websites, our hosting provider and the Google Maps embed on our contact page may collect standard technical information such as IP address, browser type, pages visited and referring site. We do not use advertising trackers or sell data.

How We Use It

We use contact-form information only to respond to your inquiry and, if you ask, to begin the hospice admission process. Form submissions are delivered to our team's secure Google Workspace account, which is covered by a HIPAA Business Associate Agreement, and are accessible only to authorized Sapphire Hospice staff.

Sharing

We do not sell, rent or share your information with third parties for their own marketing. We share information only with service providers who help us operate this website and respond to you, or as required by law.

Children
